This form is to be filled out to request any official name changes, date of birth and social security corrections, or offical gender change. This form must be returned to Kris Slaney, Registrar, Room 2112 in order to ensure this change is reflected across all University systems.

Although rarely granted, students who wish to change their clerkship rotation site after assignments have been made may only apply for a change of site if they have an extreme hardship. Requests will be considered on a case-by-case basis. Students must complete a Change of Clinical Site form and submit it to clerkship program coordinator
