Financial Aid Checklist 2008-2009

 

• Completed the Free Application for Federal Student Aid (FAFSA) for 2008-2009 before the priority service date of March 1, 2008. "Give Yourself a Valentine" and complete it by Valentine’s Day 2008 – to make the priority date, the FAFSA must be received on or before March 1^st. If the priority date has passed, you may still apply and your application will be considered for the Federal Stafford Loans.

• Applied on-line at http://www.fafsa.ed.gov/, printed out and mailed the

  signature page or signed electronically with my Personal Identification Number (PIN) AND printed the confirmation page for my records. If you don't have a PIN, go to http://www.pin.ed.gov/ to request one.

   

   

• Reviewed financial aid program information on the UA College of Medicine Financial Aid website and noted scholarships for which I want to apply.

• Completed the 2008-2009 College of Medicine Scholarship Application upon news of my admission.

• Completed the Health Professions Parental Information Form for 2008-2009 upon news of my admission. OPTIONAL:  These scholarships and loans are offered to students who are from exceptionally financially-needy backgrounds (thus, it is necessary to submit parental income regardless of student independence).

• Completed the application for the Arizona Medical Student Loan Program upon news of my admission after full consideration of the terms. OPTIONAL:  this program requires a primary care commitment to service in an eligible service site in a medically-underserved area, for a medically-underserved population, or on an Indian Reservation in the State of Arizona after residency.  Other specialties require consideration and advance approval by the Board of Medical Student Loans.

• Requested the Federal Stafford Loan from a lender after receiving the award notification AND instructions for applying from the UA Financial Aid Office (May/June 2008)
