Introduction
The University of Arizona
participates in all the federal financial aid programs and has a scholarship program
funded through private donors, foundations, and various organizations, many of whom have
set up endowed accounts. Financing a medical education is expensive.
We will assist you in
applying for aid and providing whatever support we are able with available funding.
Application forms include the following: Free Application for Federal Student Aid
(FAFSA),
Health Professions Program Parental Information Form
and
College of Medicine Scholarship Application
.
Students interested in the Arizona Medical Student
Loan
Program Application
may complete
that as well.
Free Application for Federal
Student Aid (FAFSA):
The basic application for financial
aid is the Free Application for Federal Student Aid (FAFSA). You may
apply over the internet at
http://www.fafsa.ed.gov. The earliest you may apply
for 2008-2009 is January 1, 2008 and you should plan to meet the University
of Arizona's priority service date of March 1, 2008. You may still
apply for aid after March 1; however, for consideration of all types of aid
and to have aid available by the beginning of school, you should apply as
early as possible. March 1 is a processor-received date, and we
recommend that you apply by Valentine's Day.
When you apply over the internet, you may
request a PIN number that serves as your electronic signature. If you are a continuing student on financial aid, the federal processor may
have mailed you a PIN number which serves as your signature for the FAFSA. If you don't use a PIN number, you must
print out a signature page, sign it, and send it to the federal processor when you have
submitted
your FAFSA online. Print a copy of the Confirmation Page for
your records. Your PIN number does not expire and you will be able to
use it to reapply for financial aid in future years as well as having access
to certain federal web sites. Do not give your PIN number to anyone
else. More information about the PIN is available at http://www.pin.ed.gov/
and at
http://fafsa.ed.gov/before004.htm.
If the application was processed
successfully and the processor has a valid email address, you will be sent
email notification. You may go on-line and print a copy of your Student Aid Report
(SAR) for your records. If the federal processor has no valid email
address, a paper SAR will be mailed to you.
You can check the
status of your application on the Web at http://www.fafsa.ed.gov/
from the FAFSA Follow-Up section. The information is also
sent electronically to the schools you list on the FAFSA.
If you estimated your income and need to
make corrections, follow the federal processor's instructions carefully. You are responsible
for the accuracy of the information you provided, and you should keep copies
of the income and asset documentation you used. If you purposely give false
or misleading information, you may be fined $20,000, be sent to prison, or
both!

Health Professions Programs Parental Information Form:
For federal aid programs under the Department of
Education, medical students are independent students and only the student/spouse
information is required (see Financing your Medical
Education).
For Health Professions Loans and
Scholarships, which are federal
programs administered by the Department of Health and Human Services,
parent(s)' information
must be supplied if you wish to be considered for the following:
- Primary Care Loan (PCL)
- Scholarships for Disadvantaged Students (SDS)
- Loans for Disadvantaged Students (LDS)
The Health Professions Parental Information Form
is to be completed and signed by the parent(s), who include the student for these purposes
in the Household Size (Question #66 Number of family members and #67 Number of college
students in 2008-2009). The student is the borrower if a loan is made; the
parent information is used only to determine student eligibility. If your
parents have divorced or separated, review the instructions regarding Parents' household information on including the information about a
stepparent. On the second page of the Parental Information form is the
Supplemental Information needed from medical students.
The Health Professions Training Act signed on October 13,
1992 requires a primary care service commitment for the Primary Care Loan.
You are asked on the Supplement if you wish to considered for this loan in
addition to the Scholarship for Disadvantaged Students. Submit the Health Professions Parental Information and
Supplement form to the College of Medicine Financial Aid Office.
You are advised to keep a copy for your records.
If you used estimated income information when you submitted parents'
information, submit corrections on the copy. Please have your parent(s) sign and
date the corrections and submit them to the College of Medicine Financial Aid
Office.
College of Medicine Scholarships
Students wishing to apply for need-based
scholarships should complete the
College of Medicine Scholarship Application
and return it to the College of
Medicine Financial Aid Office.
Arizona Medical Student Loan Program
(Loan repaid in service in eligible service
areas in Arizona)
Loans for the cost of tuition plus up
to $20,000 per year (e.g.
$38,508) may be available to UA College of Medicine students who are willing to
practice in an
eligible service site
in Arizona upon completion of residency. Students must intend to specialize in Family Practice,
Obstetrics/Gynecology, general Internal
Medicine, general Pediatrics, or combined Medicine and Pediatrics.
Other specialties of recognized need in Arizona require consideration and
advance approval by the Board of Medical Student Loans. As a physician
during the service commitment, the recipient must practice in a medically
underserved area, with a medically underserved population, or on an Indian
reservation located in Arizona. Funding
availability is determined yearly by the Board of Medical Student Loans based upon the
State of Arizona appropriations. Please see Arizona
Medical Student Loan Program for information and the application.
Eligibility Determination
A student must be admitted into the College
of Medicine prior to being considered for financial aid and must make satisfactory
academic progress to retain that eligibility. A student waiting to be admitted
may begin the process of applying for financial aid.
Eligibility for need-based funding is evaluated first; if you do
not qualify based on need or insufficient need-based funding is available, you may receive
an offer of non-need-based aid. A standard cost of attendance estimating the student's expenses
for registration, books and supplies, room and board, transportation and personal expenses
serves as a base; the need analysis submitted is used to determine the expected family
contribution toward those expenses. The difference between those two figures yields the
student's eligibility for need-based aid. Many federal programs and most College of
Medicine scholarships are based on need.
If your financial circumstances change significantly from that
reported for 2007 (for example, the student is working full time and quits work to attend
the College of Medicine), you may wish to discuss your options with the
College of Medicine Financial Aid Counselor.

subject to federal guidelines.
!!!WARNING!!!
BE CAREFUL OF CREDIT CARD DEBT AND LOANS
WITH MONTHLY PAYMENTS
Be careful in accruing debt, particularly loans and credit card
indebtedness that require monthly payments on top of your current educational expenses.
Car payments and outstanding debts are not allowable expenses in your
"cost of attendance" for federal financial aid. You could
become unable to handle monthly expenses.
